Reason / Defects: The plastic D-ring handle that is used to unlock the weight pocket from the BCD can detach during use and prevent the weight pocket being removed from the BCD when rising to the surface. Hazards: Risk of serious injury and death by drowning if the weight pocket cannot be removed whilst diving or in an emergency from the BCD. Incidents of the D-ring detaching on land have occurred. What to do: Stop using the weight pockets immediately.
